Requests for Assessment, Reassessment or Review (AAT)

Any adult is legally entitled to an assessment if they:

  1. Are aged 18 or above;
  2. Have an appearance of need.

If the person is aged 18 or above, you should make a record of the assessment request and transfer the contact to the most appropriate team so that a decision can be made about point b. above and, where applicable, an assessment can be arranged. If it is not clear which team this is, the contact should be transferred to the correct Locality Team.

If the person is under the age of 18, the contact should be referred to children’s services.

A carer is legally entitled to an assessment from adult care if they;

  1. Are aged 18 or above?
  2. Are providing care to a person who is also aged 18 or above?
  3. Have an appearance of need (either now or in the future)?

If points a. and b. apply, you should make a record of the assessment request and transfer the contact to the most appropriate team so that a decision about point c. above and, where applicable, an assessment can be arranged. If it is not clear which team this is, the case should be transferred to the correct Locality Team.

If the carer, or the person they support is under the age of 18, the contact should be referred to children’s services.

All reassessment or review requests should be transferred to the team with responsibility for reviewing the Care and Support Plan / Support Plan or that arranged the piece of equipment needing review. If it is not clear which team this is, the contact should be referred to the correct Locality Team.
